I hereby declare that I have reviewed the information before expressing consent to having my personal data processed and I have fully understood the information. If there had been any doubts, I was offered the opportunity to ask questions and I received satisfying answers.

Acting on my own behalf, having been previously informed about the following:
1. Personal Data Controller;
2. the purpose of data processing;
3. the possibility to withdraw the previously provided consent to the personal data processing at any time.
I consent to Polskie Wydawnictwa Specjalistyczne ProMedia Sp. z o.o with its registered seat in Warsaw (04-386 Warszawa, ul. Paca 37) (hereinafter: Controller) processing my personal data as the controller within the following scope: name and surname, telephone number, e-mail address for the following purposes:
executing the order via the Website, including within in the scope of withdrawing from the contract and considering a complaint
receiving notifications of marketing and promotional campaigns carried out by PROMEDIA
providing personal data and contact information to other attendees and conference partners

Registration is open via an electronic form or via e-mail at w.wiater@pws-promedia.pl until 6 October 2023. Receipt of your conference registration will be confirmed via e-mail. A proforma invoice will be sent to the correspondence address provided, based on which payment for attendance should be remitted. The payment deadline is 7 days from the registration date, no later than by 10 October 2023. Once payment is made, the organizer will issue a VAT invoice.

Your cancellation should be submitted in writing to w.wiater@pws-promedia.pl not later than 72hrs before the beginning of the conference. If no written cancellation is received, failure to attend does not entitle to fee reimbursement.

Zgodnie z art. 13 ust. 1 i ust. 2 ogólnego rozporządzenia o ochronie danych osobowych z dnia 27 kwietnia 2016 r. (dalej: Rozporządzenie) informuję, iż: 1. Administratorem Pani/Pana danych osobowych są Polskie Wydawnictwa Specjalistyczne ProMedia Sp. z o.o. z siedzibą w Warszawie, 04-386 Warszawa (dalej: PROMEDIA), 2. Koordynatorem ochrony danych w PROMEDIA jest: Piotr Krajewski, p.krajewski@pws-promedia.pl 3. Pani/Pana dane osobowe przetwarzane będą w celu realizacji zamówienia w serwisie internetowym: www.hotel-trends.pl, 4. Pani/Pana dane osobowe będą przechowywane przez okres: do czasu zrealizowania zamówienia, z uwzględnieniem okresu koniecznego do rozpoznania reklamacji oraz wykonania prawa do odstąpienia od umowy, 5. Posiada Pani/Pan prawo dostępu do treści swoich danych oraz prawo ich sprostowania, usunięcia, ograniczenia przetwarzania, prawo do przenoszenia danych, prawo wniesienia sprzeciwu, prawo do cofnięcia zgody w dowolnym momencie bez wpływu na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej cofnięciem; 6. Uprawnienia, o których mowa w pkt. 5 powyżej może Pani/Pan zrealizować poprzez złożenie oświadczenia o odpowiedniej treści na piśmie i przesłanie go drogą pocztową, przesyłką kurierską na adres Administratora 7. Ma Pan/Pani prawo wniesienia skargi do GIODO gdy uzna Pani/Pan, iż przetwarzanie danych osobowych Pani/Pana dotyczących narusza przepisy Rozporządzenia, 8. Podanie przez Pana/Panią danych osobowych jest warunkiem umownym skorzystania z Serwisu Internetowego.
